We will review your health history and provide information about services such as screenings, shots, and referrals for other care that is needed.

Frequently Asked Questions

Annual Wellness Visit is a yearly check in appointment, and in most cases fully covered for Medicare members. It is a chance for a healthcare provider to check in on your general health and review how things have been going related to your health.

This visit includes a review of your medical and social history related to your health, and education and counseling about preventive services such as certain screenings, shots, and referrals for other care that is needed.

During your Medicare Annual Wellness Visit, our provider will assess vital signs, review medications, medical history, diet, exercise, and preventative screenings. An annual physical requires a hands-on examination and labs may be ordered.

In most cases, Medicare will pay for the Annual Wellness Visit at 100%. There is no copay, deductible, or coinsurance for this visit. However, if other services are delivered, a coinsurance/copayment may be expected as part of those services.

You will need access to a computer, phone, or tablet with a camera/microphone to complete this visit. Instructions for the virtual visit platform are in the confirmation email sent to your email address.

You will also receive a check-in email and text the day before your appointment. Please check in for your virtual visit 15 minutes before your scheduled appointment time.

For your visit, please have the following items:

  • A list of all current medications and dosages including over-the-counter drugs, and vitamins.
  • Your preferred pharmacy information
  • The name of your Primary Care Provider and any specialists you see
  • Any medical or immunization records
  • Your family health history to help your doctor determine if you’re at risk for certain diseases
  • A reading of your most recent weight and blood pressure if available
  • A piece of paper and pen/pencil if available

You can use a laptop or desktop computer with a webcam/microphone, and a strong internet connection. We recommend that you use Google Chrome web-browser if possible.

Alternatively, you can download the myPrivia app on your mobile device with a built-in webcam/microphone and strong internet connection. The myPrivia app is supported on mobile devices (iPhone and iPad) with iOS 9.0 or later or Android 5.0 and up. If using a laptop or desktop computer with a webcam/microphone, a popup will appear in the top right hand corner of your web browser asking if you want to allow the Google Chrome to access your camera and microphone. You will need to ensure you are not using the incognito or privacy mode on your browser.

If using a mobile device with a webcam/microphone, check the device settings to allow the myPrivia app to access your camera and microphone.
