Coverage varies between plans and between states. If you have questions, please contact the Member Services Department of your health plan for specifics regarding your coverage for nutrition counseling.
When calling, we suggest verifying the following details:
- Does the insurance cover virtual nutritional counseling for codes 97802 or 97803?
- Does the insurance cover the preventive diagnosis code Z71.3?
- Are there any limitations, such as a specific number of visits per year, or a required diagnosis (e.g., Diabetes, Kidney disease)?
- Are these visits subject to a deductible? If yes, has the patient’s deductible been met for the year?
- Are there any patient-responsible co-pay or coinsurance amounts for the visit? If so, what are they?
We will work with your health plan to bill for any amount owed after your visit. You agree to be financially responsible for payment for the services provided to you. If it is later determined that your insurance coverage changed or was not active on the day of your visit, you agree to be responsible for payment of the visit.
The self-pay rate is $150 for an initial appointment and $100 for a follow-up visit.
